Case Management/Discharge Planning
The Case Manager is responsible for assess, plan and facilitates patient/family care coordination to meet their comprehensive health care needs. Collaborates with nurses, physicians, continuing care coordinators and interdisciplinary members of the health care team, as well as third party payers’ to assure a comprehensive discharge plan. Responsibilities include clinical patient evaluation and patient/family advocacy.
NYS licensed RN required. Three years Case Management or Discharge Planning experience required. Knowledge of medical insurance procedures regarding discharge planning and authorization required. Experience in Utilization Management, including the ability to conduct medical insurance reviews required. Strong assessment and communication skills required. Excellent organizational skills and ability to prioritize required. Bachelor's degree in Nursing or health-related field. NYS Screen Assessment Certification. NYS PRI Assessment Certification. Ability to communicate Spanish or community dominant language. Educated in the use of the particulate respirator (mask). Familiar with Medicaid and services provided by Medicaid. Knowledge of skilled nursing facility and rehabilitation placement. Hospice and/or palliative care experience.
