Determine patient care needs, identifies and formulates diagnoses for treatment of actual or potential problems. Documents all patient encounters and interventions according to established standards of care. Orders appropriate lab and diagnostics following established protocols. Prescribes drugs, devices and immunizing agents according to established practice agreements and practice protocols. Appropriately evaluates and interprets labs and diagnostic test results. Makes appropriate changes in plan of care in response to patient’s status, responses to treatment and his/her goals. Refers patients for specialty consultation when clinical indicated. To serve on appropriate committees in BHS and SJRH as requested. Collaborates with other members of the staff to ensure continuity of care. To participate in all UR/QA activities as requested. To perform administrative and supervisory functions as requested. Other responsibilities as assigned.


Two years experience in working with chemically dependent persons. MD with current New York State License
