The Nurse Practitioner delivers quality primary care and/or specialty care services to clients/patients who are HIV-positive, HIV-negative, Hepatitis positive, and/or others, as required by the work of the Department. Maintains and exhibits consistently positive attitudes and functions in a solution-oriented manner to facilitate the work of HOPE Center.
Provides high quality primary and specialty care services following appropriate clinical guidelines to the populations served by HOPE Center through effective participation in a multi-disciplinary setting and through coordination with the Medical Director. Appropriately orders and interprets laboratory and diagnostic tests. Assures that all tests are provided in a manner that assures reimbursement by funders and/or insurances. Prescribes drugs, devices, immunizing agents including controlled substances in accordance with established practice agreement and practice protocols. Consults with collaborating physicians when appropriate. Documents all patient encounters and interventions according to established standards of care. Provides high quality customer service to all internal and external customers (including other Departmental staff and patients). Works cooperatively & courteously with the Head Nurse, Clinical Supervisor and Director to set a positive example of inter-departmental coordination for rest of team. Routinely achieves all performance improvement goals at the levels set by the Performance Improvement Committee within her or his individual panel of patients. This shall include: Required tests for persons living with HIV, including (but not limited to) CD4/VL q 4 months, lipids, syphilis. Discussion and documentation of drug/alcohol use, sexual health, adherence counseling, required referrals (including dental, ophthalmology, and gynecology). Provision of and/or referral to gynecology services for all women. Provision of and/or referral to anal pap services, as meets treatment guidelines. Leads and/or participates in at least one performance improvement project (based on the Department’s PI plan) each calendar year. Routinely participates in and/or leads the Department’s multi-disciplinary treatment team in its weekly review of all clients receiving services at HOPE Center. Assume an active role in ensuring that HOPE Center remains in compliance with all NYS – DOH and NQCA’s Patient Centered Medical Home requirements. Documents at least two chart reviews every six months with the supervising MD to assure high quality care. Consistently assumes other duties, as required to assure efficient operation of HOPE Center. Assure that all services are delivered in a culturally and linguistically appropriate manner. This includes interfacing effectively with staff that represents other cultures. Assure that all professional licenses, including: NYS – Registered Nurse, NYS – Nurse Practitioner, DEA license are current and up to date. Copies of all current licenses are on file with the hospital’s Human Resource Department, Medical Staff Office and HOPE Center. Other duties as assigned by supervisor.
