Responsible for the administration of the THIHPS (Targeted High Impact HIV Prevention Services) program. They will be responsible for providing client centered, strengths-based services using State recommended interventions to identify and address barriers preventing clients from engaging in harm reduction, medical and behavioral health care as well as social services.
They will actively engage community partners in an effort to deliver targeted outreach activities to identified client populations. They will work to develop linkages with Community Based Organizations that will promote client referrals and engagement in needed services.
The position will:
Utilize a team approach to provide support to the client on implementing and sustaining their treatment plan.
Use evidence based intervention (motivational interviewing and social cognitive theory based interventions) to elicit information from client to complete a harm reduction care plan, assessment and reassessment of medical, behavioral and psychosocial functioning factors.
Through a collaborative effort with the client’s care team, the Prevention Supervisor will coordinate and advocate for services that will promote optimal health.
Bachelor’s Degree in Human Services, Social Work, Psychology, Public Health or Counseling; Must have at least three (3) years of work experiences in the field of HIV Prevention; At least three (3) years of supervisory experience in Prevention, Outreach, or Health Education or related field; Must have a valid driver’s license and access to personal car for work related travel; Must have computer knowledge and able to independently operate; Possesses well-developed and effective interpersonal skills and is able to communicate effectively verbally and in writing; Displays leadership ability and the willingness to assume authority and accountability for the direction and supervision of others; Possesses the ability to plan, organize, develop and implement goals, objectives, policies and procedures necessary for quality care; CASAC or Qualified Health Professional preferred; Bilingual Spanish required.
