The Nephrology nurse practitioner ( NP) works as a member of our team to help deliver quality care to our patients. NP will work collaboratively with the primary care team and Nephroloogy.
The NP will work under the supervision of the nephrology attending physicians, in a comprehensive nephrology office practice. The role will entail both direct patient care and support services, such as patient education, review of labs, discussion of results, patient follow up and medication management.
Functions independently to perform age-appropriate history and physicals Provides comprehensive primary care services, including wellness and acute illness Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities Monitors the effectiveness of interventions. Discusses results with patients Collaborates with multidisciplinary team members by making appropriate referrals Facilitates patient and family decision making by providing educational tools. Provide on call coverage as requested In addition to the Primary Care Duties, the Primary Care/Nephrology nurse practitioner will: Assist the Nephrology attending with the care needs of patients Perform detailed assessments and consultations in conjunction with the attending physician Perform patient follow-up visits Contact patients with lab and test results as needed Performs other related assigned duties. Provide on call coverage as requested Other duties as requestedPerforms unrelated duties in times of emergency.
Possession of a current valid license to practice nursing in New York State, Master of Science Degree. Certification as a Nurse Practitioner in New York State with at least one (2) year experience. Experience in nephology preferred
