Responsibilities

The Continuing Care Specialist assists in meeting the psychosocial, financial and discharge planning needs of patients and families through assessment and/or referral.

Oversees the Case Management Analyst monthly reports, including but not limited to denial reports, PI reports, and department goals.

 

  1. Educates, coordinates, and collaborates with nurses, physicians and interdisciplinary members of the healthcare team, to assure an ongoing comprehensive discharge plan.  CCC will present the Important Message from Medicare with in 2 days of discharge during regular department hours as needed.
  2. Assesses patients admitted from Adult Homes, Senior Living or Assisted Care and for patients admitted to the Intensive Care Unit and begins to develop a safe discharge plan.
  3. Receives referrals from the Emergency Department for patients with Psychosocial, financial, skilled nursing and equipment needs.
  4. Receives referrals and consults from Case Managers, RN’s and MD’s, assesses the patient and determines a safe discharge plan for patient.
  5. Counsels the patient and significant others in the admitting criteria and referral process of external agencies.
  6. Counsel’s patients and their families in relation to anxieties and stress precipitated by illness and hospitalization.
  7. Collaborates with community professionals in order to develop a safe discharge plan and maintain continuity of care.
  8. Obtains community resources in order to develop discharge plan and continuing care for the patient.
  9. Provides for patient choices when referring to skilled nursing facilities, terminal care facilities, psychiatric programs, visiting nurse referrals and substance abuse programs and provides written federal guidelines.
  10. Assists in obtaining MD orders and insurance instruction for patients post hospital continuing care needs.
  11. Assists the family members and significant others in arranging for burial of a patient.
  12.  If assigned to OB, must assess for a potential cases of child abuse and neglect and makes appropriate referrals.
  13. If assigned to OB, performs insurance reviews for maternity patients and newborns.
  14. Oversee and assist the case management analyst to make sure denial, PI, and Department goals are up to date and completed in a timely manner
  15. Escalate any reporting issues to Director of Case Management
  1. Oversees analyst with monthly denial and PI report.
  2. Oversees analyst with monthly/weekend Nursing Home reports regarding Referrals and Placements to SNF.
  3. Overseeing analyst with daily denial and EOD reports sent to the RN Case Managers.
  4. Oversees CM Assistants with Empress transports including bills, letters of medical necessity forms, and authorizations.
  5. Oversees CM Assistants with Medicaid and commercial insurance transports regarding obtaining authorizations and being an administrator for all MAS websites.
  6. Coverage for weekend reporting to Department of Health for COVID statistics.
  7. Weekday coverage for ED Status Report for Administration.

Qualifications

  • Bachelors in Social Work required.
  • Masters in Social Work preferred.
  • Knowledge of Medical Terminology.
  • Bilingual in Spanish preferred.
  • NYS Screen assessment preferred.
