The Cardiology NP may perform the initial assessment of a patient admitted to the hospital with cardiac symptoms. Based on this evaluation, the NP can determine the appropriate level of care, including indications for telemetry according to protocols, and decide whether an immediate consultation with a cardiologist is necessary or if the case can be managed by the NP team.

The Cardiology NP can be the gatekeeper for inpatient consults requested by other departments. The NP can triage these requests to determine the urgency and specialty expertise required, managing routine cases while escalating complex ones to the attending cardiologist.

As a gatekeeper, the NP coordinates the patient's care journey. This includes ordering and monitoring diagnostic tests like EKGs, stress tests, or echocardiograms and making referrals to specialists or other hospital departments, ensuring seamless care transitions.

The NP plays a critical role in facilitating continuity of care by managing discharge planning. This involves educating patients and families and coordinating follow-up care in the outpatient setting, helping to reduce readmission rates.
